Affidavit / Annexure 'C' or Annexure 'D' for Minor's Passport Application in India

With the liberalisation of passport rules, the requirement of attestation/notarisation of the Annexures has been done away with. It is therefore clarified that affidavits/annexures required for applying for a child's passport in India — in cases where one parent is in New Zealand — do not need to be attested by the Consulate General of India.

Passport applications of minor applicants may be processed by Passport Offices in India on production of self-attested Annexure 'C' or Annexure 'D', along with other required documents, as the case may be.

This service is provided by the Consulate as a facilitative measure for applicants who still wish to submit their documents through the Mission.

Documents Required

  1. Completed and signed Miscellaneous Consular Services application form.
  2. Annexure 'C' — Declaration by the applicant's parent or guardian for issue of passport to a minor when one parent has not given consent.
  3. Annexure 'D' — Declaration for minors under 18.
  4. Annexure 'E' — to be submitted by all applicants.
  5. Letter to RPO (Regional Passport Office).
  6. Payment proof — fee of NZ$21 (modes of payment).
  7. Coloured copy of the first and last pages of the current passport, attested by a Justice of the Peace.
  8. Copy of the current and valid New Zealand visa.
