• Print-out of duly filled and signed Part A and Part B of online application

• Copy of JP Attested current New Zealand / foreign passport 
• Copy of JP attested foreign Citizenship certificate 
• Copy of cancelled Indian passport (first and last page)
• Copy of JP Attested Birth Certificate of the applicant.
• Copy of JP Attested Parent’s Marriage Certificate (Apostilled / authenticated by the Government of the country of issue if marriage is registered in a foreign country).
• Surrender Certificate of the applicant(https://www.hciwellington.gov.in/page/surrender-certificate-renunciation-of-indian-citizenship/)
• Payment Proof of Fee amounting to NZD 452.

• Self-addressed courier envelope with tracking facility, if you want to receive OCI by post. Separate envelopes are to be provided for multiple applications.
