Applicants may apply for re-issue of valid passport for change in appearance subject to personal interview with the passport officer, wherever required. This will also apply to SIKH applicants who intend to go for re-issue of valid passport, renewal of passport due to change of appearance form turbanised photograph to clean shaven photograph and vice versa.  

         I.            All applicants must complete online Passport Application Form At: https://mportal.passportindia.gov.in/mission/  by creating a USER ID.  Best viewed in Internet Explorer 9.0 and above. Passport Online Application should be accurately filled.

Login using the newly created USER ID and fill in the user-friendly application form online at the same portal. 

Applicant must upload an ICAO-compliant photograph, a scanned copy of their signature and all other relevant documents prior to submitting the application (MANDATORY).

After submitting the form online, take a printout of the application.

2. Payment Proof in r/o Fee of NZ$ 126.(APPLICATIONS BY COURIER/POST: - Online Bank Transfer. IN-PERSON APPLICATIONS AT THE CONSULATE: By Debit card Only at the counter)
3. Copy of valid New Zealand visa  
4. Original Indian passport (Not attested copies please)

a.     All the submitted passports will be returned after re-issue of new passport

     V.            Please paste one photo on the Passport application. Please paste one photo on the Personal Particular form. 

a.     Do not staple/pin the photographs. 

6. Annexure "E"  must be completed and signed by the applicant.( DOWNLOAD )
7. Personal Particulars Form (DOWNLOAD ).
8. For addition of Spouse Name: Marriage Certificate Copy (If Marriage Certificate not available, Joint Photo Declaration, duly signed by both husband and wife and duly attested by JP).
9. For deletion of Spouse Name: Divorce order/Decree.
10. For Change of Spouse Name: (a) Divorce order/ decree or death certificate of first spouse and (b) Re-marriage certificate (If Re-marriage Certificate not available, Joint Photo Declaration, duly signed by both husband and wife and duly attested by JP).
11. Declaration for change of appearance -Annexure 'J' duly attested by Justice of Peace/notary.  ( DOWNLOAD ).
12. In case of Change in Address please attach any one of the following:

                                                               i.      Water/Electric/Telephone/Gas connection bill 

                                                             ii.      Valid Ration card/ Aadhar Card/ Election Commission photo ID/ Bank statement with photograph  

                                                           iii.      Parent’s passport copy.

13. Following is mandatory, if the passport has expired more than one year ago 

                                                               i.      Local Police Clearance certificate from  Ministry of Justice, New Zealand

                                                             ii.      Letter from applicant giving reason for not renewing the passport within a year of date of expiry.

14. Self-addressed prepaid courier envelope to return your passport. 

a.     The applicant may use any courier service (with tracking facility) of their choice and are advised to keep the tracking numbers with them as Consulate General of India will not take any responsibility in case the courier is lost/misplaced/damaged or delayed. Please do not send the application via ordinary post. Before making any status enquiry about the application please check with the courier service whether the application has been delivered to our office.

15. Interview with Passport Officer may be required on case-to-case basis.
