Reissue of passport for children aged between 15 to 18 years 

1. All applicants must complete online Passport Application Form At: https://mportal.passportindia.gov.in/mission/  by creating a USER ID.  Best viewed in Internet Explorer 9.0 and above. Passport Online Application should be accurately filled.

Login using the newly created USER ID and fill in the user-friendly application form online at the same portal. 

Applicant must upload an ICAO-compliant photograph, a scanned copy of their signature and all other relevant documents prior to submitting the application (MANDATORY).

After submitting the form online, take a printout of the application.

2. Fee of NZ$ 126. (APPLICATIONS BY COURIER/POST: - Online Bank Transfer. IN-PERSON APPLICATIONS AT THE CONSULATE: By Debit card Only at the counter).
3. Copy of valid New Zealand visa  
4. Original Indian passport (Not attested copies please). 

a.     All the submitted passports will be returned after re-issue of new passport.

5. Annexure "E"  must be signed by both parents ( DOWNLOAD ). 
6. Both parents' passports copy (Bio Page and back details page) attested by Justice of Peace.
7. Both parents' New Zealand visa copy attested by Justice of Peace. 
8. Annexure "D"  must be signed by both parents ( DOWNLOAD ). 
9. In case one parent has not given consent for issue of passport for the child,  Annexure - "C" must be completed. ( DOWNLOAD ). 
10. Personal Particulars Form ( DOWNLOAD ). 
11. In case of Change in Address please attach any one of the following:

a.)         Water/Electric/Telephone/Gas connection bill 

b.)         Valid Ration card/ Aadhar Card/ Election Commission photo ID/ Bank statement with photograph  

c.)         Parent’s passport copy. 

12. Following is mandatory, if the passport has expired more than one year ago 

a.)         Local Police Clearance certificate from  Ministry of Justice, New Zealand

b.)         Letter from applicant giving reason for not renewing the passport within a year of date of expiry.

13. Self-addressed prepaid courier envelope to return your passport. 

a.                  The applicant may use any courier service (with tracking facility) of their choice and are advised to keep the tracking numbers with them as Consulate General of India will not take any responsibility in case the courier is lost/misplaced/damaged or delayed. Please do not send the application via ordinary post. Before making any status enquiry about the application please check with the courier service whether the application has been delivered to our office.

14. Interview with Passport Officer may be required on case-to-case basis.
