When to apply for re-issuance of OCI card:

• Issuance of new passport after completing 20 years of age;
• Change of personal particulars viz. nationality etc;
• Loss/ damage of OCI registration certificate;
• Filling of wrong personal particulars while submitting online application viz. name, father's name date of birth etc.
• Change of address/occupation of the OCI cardholder.

Document Requirement:

• In case of issuance of new passport: A copy each of (i) OCI card, (ii) new passport and (iii) old passport. If the old passport is not returned by the concerned authorities while issuing new passport, copy of old passport may not be enclosed.
• In case of loss of OCI card: A copy of the complaint lodged with Police authorities/ foreign police department (in case of theft/loss/ damage of OCI card) and a copy of the OCI card. If the report is in foreign language, authorized translation is required. If a copy of the OCI card was not taken prior to the loss, it may not be enclosed.
• In case of change of personal particulars: Copy of the documents relevant for effecting the requisite changes. Sufficient proof for each requested services like change of name, change of address, occupation etc. is to be furnished
  Address proof - electricity bill or landline phone bill or undertaking letter of concerned person or rent/ lease agreement.

Evidence of address of the place of application

• Copy of the Electricity Bill/Telephone bill in own/parents/ grandparents/ spouse name.
• If the above is in any other relative's name, undertaking from the relative stating that the foreigner is residing with him/her.
• Lease License in case of tenants.

Procedure:

Step1: 

Go to the Website : https://ociservices.gov.in/welcome  - Click on Miscellaneous application for reissue of OCI card 

Step 2:

Fill new application (enter your OCI/PIO card, passport number linked to OCI/PIO card and the date of birth to proceed further)

Payment Proof of Fee payment amounting to NZD 46 - Reissue of OCI card

• Specification guidelines for uploading of documents: 

The maximum size allowed for a document file is 500 kb. The document file must be in PDF format. (For sake of comparison, an A-4 size page document color scanned at 100 dots per inch resolution will generate a file of about 200-250 kb). Each category of document must be uploaded in a single file.

• Guidelines for uploading of photo: 

The image must be in JPEG or JPG format with maximum size of 200 KB. The minimum resolution of the photo must be 360 pixels (width) X 360 pixels (height) and the maximum resolution must be 900 pixels (width) X 900 pixels (height). The file extension should be NAME.JPEG while uploading it.

• Guidelines for uploading the signatures: 

The signatures must be in jpeg or jpg format with maximum size of 200 KB. The height and width of the signature photo must have aspect ratio of 3:1. The minimum resolution of the signature photo must be 360 pixels (width) X 120 pixels (height) and the maximum resolution must be 900 pixels (width) X 300 pixels (height).

• Physical documents required to be sent to the Consulate General of India Auckland after uploading them:

• One copy of the duly signed application form for each applicant.
• Physical OCI Card needs to be sent along with the application 
• Declaration by the OCI applicant, please click (Link to the form)
• JP attested copies of old passport and new passport
• JP attested copy of Marriage certificate, if required for adding spouse name.
• Name change documents in case of change of name on new passport.
• Copy of Police Report along with an explanatory letter in case of lost / stolen / misplaced OCI card. The police report must mention the OCI card number.
• For removing spouse name, a supporting document should be provided, duly attested by Justice of Peace.
• Prescribed fees of NZ $46 in case of reissue of OCI card and NZ $168 in case of loss / damage of OCI card/Conversion of PIO to OCI card. (The fee shall not be refunded if the application is rejected for whatever reasons.)
• Two photographs of size 2 inches X 2 inches with light background, showing full frontal face and shoulders covering 80% of the picture. Photograph should NOT be stapled or signed.

• Self-addressed registered courier envelope with tracking facility for returning the new OCI card once it is ready.
